Can someone please explain to me why my infantry in buildings seem ineffective but the AI's seem very effective. I am also keen to know what the skull means when placed over the enemy I used to know...

Depends on how much infantry you have in a building. This is because the infantry rotate around the various firing slots (windows etc). You can see this by their changing fields of vision.
Best tactic is to have another friendly unit (tank) close to the building so that their field of vision is constant and thus can be able to fire on any approaching enemy.
This rotation in buidlings is hard coded in the original Blitz game, but is more apparant in OB because if the increased ranges.

shame you cant set it to a specific angle as mine seem pretty ineffective at present

When attacking enemy troops in a building, it's best to storm 2 or 3 surrounding buildings simultaneously, so you get your troops shooting from all sides at the enemy. Use smoke rounds from a mortar to hide your troops as they dash for the doorway. ;D
That way you gain numerical superiority, and win quickly.

You will find the Inf In buildings and trenches are hard to dislodge, put more than one platoon on the job helps, with as Wolfgang says: smoke.
